Error Category 5: Missing Mandatory Readbacks

The Trap: Some clearances require readback by aviation regulation, but pilots sometimes omit them or respond with a simple "Roger."
Mandatory readback items:
  • Hold-short instructions
  • Runway crossing instructions
  • Altitude assignments
  • Speed assignments
  • Approach clearances
  • Takeoff clearances
Examples:
Issued: "OE-DMS, hold short runway two-four." Wrong response: "Roger, holding." Correct response: "Hold short runway two-four, OE-DMS."
Rule: Always read back altitude, heading, speed, and runway clearances. If ATC wants only a simple acknowledgment, they will say "acknowledge."
